Sinus pressure behind the eyes: what to know

Inflammation or mucus swelling around the sinus spaces near the nose and forehead can create fullness, tenderness, congestion, or pain that seems to sit behind the eyes. A cold can trigger this, and sinusitis can continue after other cold symptoms change. A migraine may cause pain around the eye with nausea or sensitivity to light, even without a blocked nose. Notice whether symptoms affect one or both sides, worsen when bending forward, or occur with nasal discharge, fever, visual symptoms, or light sensitivity.

Rest, drink fluids, and use a warm compress over the face if it feels soothing. Saline nasal spray or a gentle saline rinse may loosen mucus; use sterile, distilled, or previously boiled and cooled water for a rinse. Avoid smoke and other irritants. Do not share nasal sprays, and follow the product directions.

Seek urgent care for swelling or redness around an eye, severe eye pain, reduced or double vision, confusion, a stiff neck, or a sudden severe headache. Arrange a medical assessment for persistent or worsening pressure, repeated episodes, fever with marked facial pain, or symptoms that improve and then become worse. These features need an examination rather than assuming the problem is simple sinus pressure.
