Silent chest asthma: symptoms and urgent action

Asthma narrows and inflames the airways. Wheezing can occur when air passes through narrowed passages, but a very severe narrowing may leave too little airflow to make much sound. Signs can include fast or shallow breathing, chest tightness, difficulty speaking, unusual sleepiness, and visible effort around the ribs or neck. A cough may occur, but its absence does not exclude a serious flare. Asthma can coexist with allergic rhinitis, house dust mite allergy, or chronic cough, so a clinician may look for several contributors.

Sit upright, keep calm as far as possible, and follow your prescribed asthma action plan. Use your reliever inhaler with a spacer if your plan recommends one. If breathing remains difficult, the medicine is not working as expected, or the chest becomes quiet while you are struggling, call emergency services. Do not lie flat or travel alone for severe symptoms. Clinicians may consider nebuliser treatment alongside other urgent care.

Call for emergency help if you cannot speak normally, become confused or faint, develop blue or grey lips, or feel too exhausted to breathe effectively. Arrange prompt follow-up after recovery to review preventer treatment, triggers, and inhaler technique. A quiet chest should never be used as a reason to delay assessment.
