How does traction therapy work?

Short answer

Traction therapy works by applying a controlled pulling force that gently separates or unloads spinal segments, which may ease pressure and make movement more comfortable.

This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.

At a glance

Main action
Controlled pulling force
Common focus
Neck or lower back
Goal
Temporary unloading and easier movement

What the treatment involves

A clinician positions you on a treatment surface or in a supported seat, then chooses the direction, force and timing of the pull. Traction may be delivered by hand or with equipment, and it can focus on the neck or lower back. The aim is to reduce compressive loading temporarily and allow easier movement; it does not physically stretch every painful structure or correct every cause of pain.

Your response guides the session. A mild pulling sensation can be expected, but sharp pain, new tingling or worsening symptoms should be reported straight away. Traction may be considered alongside exercise, posture work and other care for problems such as [Neck Pain](/symptoms/neck-pain/), including symptoms associated with [Cervical Spondylosis](/conditions/cervical-spondylosis/).

How to prepare and participate

Tell the clinician about previous spine surgery, osteoporosis, numbness, weakness, dizziness and any condition that affects your joints or circulation. Wear comfortable clothing and follow the advised position during treatment. Do not increase the pulling force or copy a clinic setup at home without specific guidance. Ask whether gentle exercises should follow the session and whether a [Spinal Decompression Table](/technology/spinal-decompression-table/) or [Non-Surgical Spinal Decompression](/treatments/manual-and-manipulative-therapy/non-surgical-spinal-decompression/) is being discussed, since these terms can describe different approaches.

When to seek care

Stop the session and seek prompt medical advice if pain becomes severe, or if you develop new weakness, spreading numbness, loss of balance or changes in bladder or bowel control. Urgent assessment is needed for sudden major weakness, loss of sensation around the groin or difficulty controlling urine or stool. Persistent neck or back pain also deserves evaluation when it follows a significant injury or keeps limiting ordinary activities.

Questions for your doctor

What is traction intended to change in my case? How will you adjust the force if symptoms increase? Should traction be combined with exercise, and what signs mean I should stop?

You may feel a gentle pulling sensation, but sharp or increasing pain should be reported and may mean the settings need changing.

Can traction replace exercise?

Usually it is considered as one part of care; a clinician can explain whether exercise or another treatment is also needed.
