What is cervical spondylosis?

Cervical spondylosis is wear-related change in the neck discs and joints that may cause pain, stiffness or pressure on nerves.

It is also called neck arthritis or degenerative neck disease. Changes can be visible on imaging without causing symptoms, so diagnosis combines scan findings with your examination and daily experience.

Symptoms

Neck pain and neck stiffness are common presentations of cervical spondylosis. Discomfort may spread toward the shoulder or upper back and may be worse after holding the head in one position. Arm numbness and tingling can occur when a nerve root is affected; hand clumsiness, balance changes or leg symptoms need prompt review because they can indicate spinal-cord involvement.

Causes

The discs between neck bones gradually lose height and flexibility, while the small joints may develop arthritic changes. The body can form extra bone around stressed joints. These changes may narrow spaces for nerve roots or the spinal cord, although narrowing does not always produce symptoms.

Course and progression

Symptoms often vary over time, with flares after prolonged posture, strain or poor sleep. Many people manage symptoms with movement and non-surgical care. New arm weakness, hand clumsiness or balance difficulty changes the urgency and requires clinical assessment.

Risk factors

Age, previous neck injury, repetitive loading and long periods in one posture can contribute. Jobs or hobbies involving vibration, overhead work or frequent neck extension may add mechanical stress. Smoking and low physical activity may also affect spinal health.

Diagnosis

A clinician asks about pain, stiffness, arm symptoms, hand function and balance, then checks neck movement, reflexes, sensation, strength and walking. X-rays or MRI may help show disc, joint, nerve or spinal-cord changes when the examination indicates a need. Imaging findings are interpreted alongside symptoms rather than treated in isolation.

Treatment options

Physiotherapy can build neck and upper-back strength, improve movement and teach load management. Manual Therapy or Traction Therapy may be suitable for selected symptoms. Medication may help some people, and a Facet Joint Injection can be discussed when pain appears to come from those joints. Cervical Fusion is considered when structural compression and symptoms remain significant despite appropriate care.

Living with cervical spondylosis

Use frequent movement breaks, adjust screens to reduce prolonged neck bending, and keep walking or other comfortable activity in your routine. Build exercise gradually and note whether arm sensation, hand control or balance changes, rather than focusing only on pain intensity.

Possible complications

Nerve compression may cause persistent arm pain, numbness or weakness. Spinal-cord compression can affect hand coordination, balance and walking. Prompt assessment is needed if these functions worsen, or if bladder and bowel control changes.

Prevention

You cannot remove normal ageing changes, but you can support neck function with regular strengthening, varied posture, safe lifting and recovery from repetitive tasks. Treat recurring stiffness early when it begins to restrict movement or sleep.

Finding a specialist

An orthopedist, spine specialist, neurologist or physiotherapist can help match symptoms to examination findings. Describe any arm sensation changes, hand coordination problems or balance issues clearly at the visit.

Questions people ask

Does cervical spondylosis always cause pain?

No. Neck changes may appear on imaging without symptoms. Pain, stiffness or nerve symptoms matter when deciding whether treatment is needed.

What symptoms need prompt review?

New or worsening arm weakness, clumsy hands, balance difficulty or walking changes should be assessed promptly because they may reflect pressure on the spinal cord.
