Ferritin test for hair loss: what to expect

Short answer
A ferritin test for hair loss checks stored iron in the blood and can identify iron depletion as a possible contributor, while the hair pattern still determines what else should be assessed.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.
At a glance
- Sample type
- Ferritin is measured from a blood sample.
- Best interpretation
- The result is read with symptoms, other blood results, and the scalp pattern.
- Treatment caution
- Iron supplements should follow a clinician’s assessment of the result and cause.
Why a clinician may request it
Diffuse shedding can follow a period when the body has been under strain or has less available iron. Ferritin provides a picture of stored iron, which is different from measuring only the iron circulating at the moment of the blood draw. Results are interpreted with the laboratory range and with haemoglobin, symptoms, diet, menstrual history, pregnancy, chronic illness, and possible blood loss.
The test is not a stand-alone diagnosis for hair loss. A normal result does not exclude female pattern hair loss, traction alopecia, alopecia areata, telogen effluvium from another trigger, or scalp ringworm. A low result may be relevant without being the sole explanation. The clinician will also look at whether hairs are shedding from all over, becoming finer in a pattern, breaking, or disappearing in defined patches.
Preparing and following up
Ask the ordering clinician whether you need any preparation; many blood tests require no special hair-related preparation, but instructions can differ when several tests are ordered together. Tell the clinician about supplements, medicines, dietary restrictions, heavy periods, recent illness, and any bleeding. Do not start iron simply because hair is shedding. If depletion is confirmed, treatment and follow-up should address both replacement and the reason stores are low. Keep a record of shedding, scalp symptoms, and photographs so later changes can be compared with the test result.
When testing should not be delayed
Book a medical review when shedding persists, accelerates, or leaves visible thinning, especially with fatigue, breathlessness, dizziness, paleness, heavy menstrual bleeding, or dietary difficulty. Seek prompt attention for fainting, severe weakness, significant bleeding, or a painful, infected-looking scalp. A rapidly expanding smooth patch also warrants timely examination because a blood test alone cannot identify its cause.
What to ask at the visit
What question is the ferritin test meant to answer in my case? Which related blood results should be reviewed together? If ferritin is low, how will we look for the reason and monitor treatment? If it is within range, what does my scalp pattern suggest, and what is the next step?

Questions people ask
Does a ferritin test diagnose the cause of hair loss?
No. It can identify iron depletion as one possible contributor, but examination and history are needed to identify the hair-loss pattern.
Should I fast before a ferritin test?
Follow the instructions for the full group of tests ordered. The clinic can tell you whether fasting is needed for your appointment.
