Dandruff behind the ears: what to know

The skin behind the ears can be affected when scalp dandruff or seborrheic dermatitis extends beyond the hairline. It may look flaky, greasy, red, or cracked and can itch under hair, glasses, helmet straps, or hearing devices. Shampoo residue, hair products, sweat, and friction can irritate the fold. A sore rash with crusting or discharge is different from simple loose scale and may need a closer examination.

Rinse shampoo thoroughly and dry the fold by patting rather than rubbing. Keep hair and device straps clean, avoid picking, and pause products that sting. A medicated shampoo may help when scalp dandruff is part of the problem; use it according to directions and keep it out of the ear canal. If a clinician identifies a suitable cause, topical antifungal treatment may be discussed. Do not apply steroid or antifungal creams inside the ear without guidance.

Arrange medical advice for persistent itch, swelling, cracks, bleeding, pus, bad smell, or pain behind the ear. Seek prompt review if redness spreads, the ear becomes very tender, or hearing changes occur. Recurrent scale despite appropriate scalp care may indicate seborrheic dermatitis, contact irritation, psoriasis, or infection rather than routine dandruff.
