COVID and GERD: what should you know?

COVID-19 and GERD can overlap in cough or chest discomfort, but their patterns and treatments are different. COVID-19 may bring fever, sore throat, tiredness, or a new change in smell or taste, while reflux often causes burning behind the breastbone, a sour taste, or symptoms after meals and when lying down. One condition does not prove the other.

Reflux happens when stomach contents move upwards into the oesophagus. It can irritate the throat and cause cough, hoarseness, or disturbed sleep. A COVID-19 infection can also cause cough and fatigue, and being unwell may change eating, sleep, and medicine routines in ways that aggravate reflux. Chest symptoms should not automatically be labelled GERD: a new or severe pressure, especially with breathlessness, sweating, or weakness, needs urgent assessment.

If you have respiratory symptoms, follow current testing and infection-control advice. For suspected reflux, eat smaller meals, avoid lying down soon after eating, and notice whether particular foods, alcohol, or late meals trigger symptoms. Keep hydrated and take prescribed medicines as directed; do not add or stop acid-reducing treatment without advice. Write down when symptoms occur, what you ate, your position, fever, and any breathing changes so a clinician can compare the patterns.

Get emergency help for severe or persistent chest pressure, marked breathlessness, blue lips, fainting, confusion, or pain spreading to the arm, jaw, or back. Arrange prompt review for difficulty swallowing, vomiting blood, black stools, unexplained weight loss, repeated vomiting, or a cough that does not settle. A clinician can assess whether symptoms fit acid reflux, COVID-19, another respiratory illness, or a heart problem.

Ask whether your symptoms need COVID-19 testing, reflux treatment, or an examination for another cause. Mention any hiatus hernia, weight changes, medicines, night-time symptoms, and response to previous treatment. Ask which warning signs should bring you back urgently and how to take your regular medicines if infection reduces your appetite.
