Coughing up blood from vaping: what to know

Small streaks can appear when forceful coughing irritates the throat or bronchial lining. Vaping can expose the airways to substances that trigger inflammation, but blood should not be dismissed as a normal vaping effect. Infection such as pneumonia, longer-term airway damage such as bronchiectasis, or a clot affecting the lungs can also cause haemoptysis. Blood from the nose, gums, or mouth can sometimes be mistaken for blood from the chest.

Stop vaping while you arrange clinical advice, and avoid smoking or inhaling other products. Note the colour and approximate amount, whether it was mixed with mucus, and when it happened. Tell the clinician about the device, liquids or substances used, recent illness, medicines, and any history of lung disease. Do not start leftover antibiotics or use cough medicines to hide a worsening symptom.

Call emergency services or go to an emergency department for more than a trace of blood, repeated bleeding, shortness of breath, chest pain, a fast heartbeat, faintness, confusion, or blue-grey lips. Urgent assessment is also needed if you feel suddenly unwell after a long journey, recent operation, or period of reduced movement, because a pulmonary embolism is one possible cause.

Ask what source of bleeding is most likely, whether you need a chest examination or imaging, and which vaping ingredients may have irritated your airways. Ask how to monitor the symptom and what changes should prompt immediate care.
