What should you know about cardiac asthma?

The name can be confusing: cardiac asthma is not a separate form of airway asthma. The heart may be unable to pump efficiently, raising pressure in the blood vessels around the lungs. Fluid can then collect in lung tissue and make breathing feel tight or noisy, especially when lying down.

People may develop a persistent cough, wheezing, rapid breathing, tiredness, or shortness of breath during activity or at rest. Waking breathless at night, needing extra pillows, or noticing swelling around the ankles can point towards fluid retention and should be assessed.

These symptoms can overlap with asthma, a respiratory infection, or other causes of chest tightness. A clinician may listen to your chest, check oxygen levels and heart signs, and arrange tests such as a chest image, heart tracing, or blood tests. Do not assume that wheezing is ordinary asthma, particularly if it is new in adulthood or occurs with ankle swelling.

Arrange prompt medical assessment for new or worsening breathlessness, a cough that is worse when lying down, or wheezing that does not fit your usual pattern. Bring a list of medicines and mention any known heart, kidney, or lung condition.

Treatment addresses the heart problem and excess fluid. A clinician may adjust prescribed heart medicines or recommend other treatment after examining you. Do not start, stop, or increase water tablets or inhalers without advice. A nebuliser treatment may be used in urgent care for selected breathing problems, but it does not establish that the cause is asthma.

Seek emergency help if severe breathlessness comes on suddenly, you cannot speak in full sentences, your lips look blue or grey, you become confused or faint, or you have chest pressure with sweating or nausea. Sit upright while waiting and avoid driving yourself.

Urgent assessment is also needed when breathing rapidly worsens, you cough up pink or foamy fluid, or symptoms prevent you from lying down. These signs can reflect serious fluid build-up in the lungs or another emergency, so they should not be judged by how similar they feel to a previous asthma episode.

Ask what is causing your breathing symptoms and which findings support that explanation. You can also ask whether you need heart or lung tests, how to monitor weight or swelling, which symptoms should trigger emergency help, and how each prescribed medicine should be taken.
