Asthma and losing your voice

The voice is made in the larynx, above the airways affected by asthma. When both occur, each problem should be assessed on its own features rather than assuming that poor asthma control explains every voice change.

Repeated coughing can strain the vocal folds. A cold, allergy-related nasal drainage, acid reaching the throat, smoke exposure or heavy voice use may also inflame them. Asthma commonly presents with cough and wheeze, and forceful coughing during a flare can temporarily change the voice.

Inhaled corticosteroid medicine may contribute to a rough or weak voice when medicine settles around the larynx. White patches, soreness or an unpleasant taste can suggest oral thrush, which needs clinical assessment. Do not stop controller treatment without discussing an alternative or technique adjustment.

Rest the voice, speak gently rather than whispering, drink fluids and avoid smoke. Follow your asthma plan for cough, wheeze, breathlessness or chest tightness. After an inhaled corticosteroid, rinse and spit as instructed; a spacer may reduce throat deposition when it is suitable for that inhaler.

Book an examination if hoarseness keeps returning or does not settle after the apparent trigger passes. Take all inhalers to the appointment so the clinician can check the dose, device and technique. Mention reflux symptoms, nasal symptoms, smoking or vaping, and any work that demands prolonged speaking.

Seek emergency help if voice loss occurs with severe breathlessness, noisy breathing from the throat, inability to swallow saliva, swelling of the lips or tongue, blue or grey lips, confusion, or asthma symptoms that worsen despite rescue treatment. These signs can indicate a threatened airway.

Arrange prompt review for coughing up blood, a neck lump, pain when swallowing, unexplained weight loss, or persistent hoarseness, particularly with a history of smoking. These features need examination of the throat and larynx.

Ask whether the voice change could relate to inhaler technique, thrush, reflux, nasal drainage or vocal strain; whether your device can be used with a spacer; and when examination by an ear, nose and throat clinician would be appropriate.
