Coughing during an asthma attack

The cough may be dry or accompanied by phlegm and can worsen at night, with exercise, during a respiratory infection, or after exposure to smoke, dust or cold air. Some flares also cause wheeze, chest tightness and shortness of breath.

Coughing is a protective reflex, but inflamed asthma airways can trigger it too easily. A cough that appears with familiar triggers and improves as asthma treatment takes effect may fit an asthma flare. Cough alone, however, can have many causes.

Keep track of night waking, exercise limitation, fever, nasal symptoms, phlegm and reliever use. This pattern can help distinguish poor asthma control from an infection, upper-airway irritation or another reason for persistent cough.

Move away from a suspected irritant, stop strenuous activity and sit upright. Use your prescribed reliever according to your asthma action plan, checking that the inhaler and spacer are assembled and used as you were taught.

Small sips of water may soothe throat irritation, but drinks and cough remedies do not treat narrowed asthma airways. Seek a routine clinical review if the cough keeps returning, wakes you, interferes with speech or exercise, or requires reliever treatment more often than your plan anticipates.

Call local emergency services if coughing comes with severe breathlessness, gasping, blue or grey lips, confusion, drowsiness, collapse, or inability to speak comfortably. Get urgent help if the person is deteriorating or their prescribed reliever is not giving the expected benefit.

A child who becomes unusually quiet, exhausted or less responsive during breathing difficulty needs emergency attention; the absence of a loud wheeze does not make the episode safe.

Ask whether the cough pattern suggests uncontrolled asthma, how to adjust treatment under the written plan, and whether nasal allergy or infection is contributing. Have the clinician watch your inhaler technique and clarify what response should trigger same-day or emergency care.
