Anaphylaxis and asthma: recognising an emergency

People with asthma should not assume sudden breathing trouble after food, medicine, an insect sting, or another allergen is only an asthma flare. Throat swelling, a hoarse voice, widespread hives, faintness, or collapse point to a broader allergic emergency.

Asthma narrows the lower airways, leading to cough, wheeze, chest tightness, and shortness of breath. Anaphylaxis can also narrow the airways, but may additionally swell the tongue or throat and disrupt blood circulation. Skin changes can occur, yet their absence does not rule out anaphylaxis.

Anaphylaxis presents with wheezing in some people, especially when the lungs are involved. That statement describes how the condition can present; wheezing itself has many possible causes, including an ordinary asthma exacerbation. The exposure and the full pattern of symptoms guide the emergency response.

Use an adrenaline auto-injector without delay when anaphylaxis is suspected, then call emergency services. Lie the person flat and raise the legs, unless breathing difficulty makes a supported sitting position necessary. Keep them still and follow their emergency plan while help is coming.

An asthma reliever inhaler may be used for persistent wheeze after adrenaline according to the person’s plan, but it cannot treat throat swelling or circulatory collapse. A nebuliser may support asthma treatment in clinical care; it does not replace adrenaline for anaphylaxis.

Call emergency services for sudden difficult or noisy breathing, tongue or throat swelling, inability to speak normally, blue or grey lips, severe dizziness, confusion, fainting, or collapse. Treat these signs as anaphylaxis when they follow a likely allergen exposure, even without a rash.

Emergency evaluation remains necessary if symptoms improve after adrenaline. Breathing difficulty that does not follow an obvious allergen exposure also needs urgent help when the reliever inhaler is ineffective, speech is limited, exhaustion develops, or the person’s asthma plan identifies the episode as severe.

Review how your asthma and anaphylaxis plans work together. Clarify which symptoms call for adrenaline, when to add the reliever inhaler, how to check auto-injector expiry and technique, and whether asthma control needs adjustment. Ask that family, school, or workplace contacts understand both plans.
