Allergy challenge tests: what to expect

It is often considered when your history and earlier allergy tests do not give a clear answer. Because a reaction can occur, the challenge is conducted where trained staff can observe you and provide treatment promptly.

A challenge tests whether exposure causes symptoms, while a skin or blood test mainly detects sensitisation. Sensitisation means the immune system recognises a substance; it does not by itself prove that normal exposure will cause illness.

The substance may be given by mouth, inhaled, or applied in another controlled way depending on the suspected allergy. Staff increase exposure in planned stages and check for skin, breathing, digestive, or circulation changes. A clinician interprets the result alongside your previous reactions and medicines.

Follow the testing service's instructions about eating, current illness, and medicines. Some antihistamines can mask symptoms, but do not stop any prescribed medicine unless the testing team specifically tells you to. Bring your medication list and describe the suspected trigger, the amount involved, how quickly symptoms began, and what treatment you needed.

Allow time for observation after the last exposure. Arrange transport if the clinic advises it, and ask what symptoms should delay the appointment, such as an asthma flare or fever.

During the test, report itching, hives, coughing, throat tightness, nausea, dizziness, or any breathing change immediately rather than waiting for the next check. Staff may stop the challenge and treat the reaction.

After leaving, use your emergency plan and seek emergency help for trouble breathing, swelling of the tongue or throat, collapse, severe dizziness, or rapidly worsening symptoms affecting more than one body system. If you carry adrenaline, use it as instructed and still obtain emergency care.

Ask which substance and route will be used, which medicines need adjustment, how long observation is expected, and what a positive, negative, or inconclusive result would change. Confirm what to do if you become unwell before the appointment and whether you should bring your own prescribed rescue medicines.
