Adenosine and asthma: breathing effects and precautions

“Adenosine asthma” is not usually the name of a separate asthma type. The phrase may refer to bronchospasm after medically administered adenosine or to an airway challenge that uses a related substance to assess airway sensitivity. The purpose and precautions differ, so confirm which context applies.

In asthma, airway lining and surrounding muscle can respond strongly to certain signals. Adenosine may trigger release of substances from airway cells, leading to muscle tightening, cough, wheeze or chest tightness. Asthma commonly presents with wheeze, cough, breathlessness and variable chest tightness, but the same symptoms can have other causes.

A clinician interpreting an airway challenge considers symptoms together with measured breathing changes. Adenosine given into a vein for a heart-related reason is a different procedure and requires the treating team to assess respiratory history beforehand.

Tell the clinical team about asthma, recent wheezing, current chest infection, previous reactions to adenosine and every inhaler or breathing medicine you use. Follow the testing service's instructions about medicines; withholding an inhaler without specific directions can worsen control or distort results.

Keep your reliever inhaler available as directed and follow your written asthma action plan. If symptoms are recurring outside a planned procedure, arrange an asthma review to check inhaler technique, adherence, triggers and whether the treatment plan remains suitable.

Seek emergency help for severe breathlessness, difficulty speaking in full sentences, blue or grey lips, confusion, exhaustion, collapse, or symptoms that continue to worsen despite using the prescribed reliever. During a supervised procedure, report cough, wheeze, throat discomfort or chest tightness immediately so the team can stop and assess you.

A mild brief sensation after a medicine still deserves mention to staff because they need to distinguish an expected effect from airway narrowing or another reaction.

Ask why adenosine is being considered, whether your current asthma control changes the plan, which inhalers to take on the day, and how breathing will be monitored. If an alternative is offered, ask how it answers the same clinical question and what preparation it requires.
