Adenomyosis on ultrasound: what to know

Adenomyosis occurs when tissue resembling the uterine lining is found within the muscular wall of the uterus. On ultrasound, a clinician may describe an uneven or bulky uterine wall, small cyst-like spaces in the muscle, fan-shaped shadowing, or a blurred boundary between the lining and muscle. Not every feature is present, and similar appearances can occur with fibroids or other uterine conditions.

A transvaginal ultrasound usually gives a closer view of the uterus than an abdominal scan. The person performing it should explain the procedure and seek consent. A normal or uncertain scan does not by itself settle the cause of ongoing painful periods, pelvic pain, or troublesome bleeding.

Review the written report with a gynaecology clinician and describe when pain or bleeding occurs, how it affects daily activities, and whether pregnancy is a current or future goal. Keeping a cycle and symptom record can reveal patterns that a single scan cannot show.

Management is based on symptom burden and personal priorities, not the image alone. Options may include pain relief, medicines that reduce bleeding, hormonal treatment such as a hormonal coil, or a procedure for selected patients. Surgery that removes the uterus ends the possibility of carrying a pregnancy, so fertility plans belong in the discussion before treatment is chosen.

Arrange a clinical appointment if pelvic pain, painful periods, or bleeding repeatedly disrupts sleep, work, exercise, or sex. Seek urgent care for sudden severe pelvic pain, fainting, marked dizziness, breathlessness, or bleeding that is rapidly soaking through period products. New pelvic pain with a positive pregnancy test also needs urgent assessment because adenomyosis should not be assumed to explain it.

Ask which ultrasound features support the impression, whether fibroids or endometriosis could also fit the symptoms, and whether further imaging would change management. It is also useful to ask how each treatment may affect bleeding, pain, contraception, and plans for pregnancy.
