What painful periods can feel like
Period pain may be cramping, gripping, aching or stabbing in the lower abdomen, with pain spreading to the back or thighs. It can start before bleeding, peak during the first part of a period, or continue beyond it. Nausea, loose stools, headache, fatigue or pain with sex may occur alongside cramps. Record when pain begins, how long it lasts and whether it is changing from your usual pattern.
Painful periods can involve cramping from uterine contractions or an underlying pelvic condition, and the timing, severity and associated symptoms guide assessment.
When to seek urgent care
Seek urgent medical help for sudden severe pelvic pain, faintness, heavy bleeding, fever, vomiting, or possible pregnancy with pain or bleeding. Arrange a clinical appointment when pain stops normal activities, is new or worsening, occurs between periods, or does not improve with your usual measures. Urgency depends on warning symptoms and severity, not on whether period pain is familiar.
Possible causes
The causes below are alphabetical, not a likelihood ranking. Adenomyosis can present with painful periods and a tender or enlarged uterus. Endometriosis can cause period pain, pain with sex or bowel and bladder symptoms around menstruation. Ovarian Endometrioma can present with pelvic pain and painful periods. Pelvic Inflammatory Disease can cause pelvic pain and abnormal discharge. Uterine Fibroids can present with painful or heavy periods, depending on their size and position.
How painful periods are diagnosed
Your clinician will ask about cycle timing, bleeding, pain location, bowel or bladder symptoms, sexual pain, medicines and pregnancy possibility. They may examine the abdomen and pelvis with your consent. An ultrasound can assess the uterus, ovaries and other pelvic structures; additional tests depend on the findings. A pain diary showing cycle days, bleeding and activities affected can make the appointment more useful.
Treatment options by cause
Anti-inflammatory Therapy for Period Pain may reduce cramping when started as advised around the beginning of symptoms. Hormonal options such as the Combined Pill or Hormonal Coil can reduce bleeding and period-related pain for some people. Endometriosis, adenomyosis, fibroids or an ovarian endometrioma may require specialist medical or surgical care. Pelvic inflammatory disease needs prompt treatment directed at the infection. Your clinician should consider pregnancy plans, other conditions and medicines before choosing therapy.
Ways to reduce period pain
Use a heat pack on the lower abdomen or back, keep gentle movement if it feels comfortable, and take recommended pain relief early in the episode. Regular sleep, hydration and tracking your cycle can help you plan. Do not ignore a pattern that is becoming more limiting: early assessment can identify a treatable cause and prevent repeated disruption to work, study or daily activities.
Symptoms that may travel with cramps
Mention heavy bleeding, bleeding between periods, pain with sex, pain passing stool or urine, bloating, infertility concerns, fever, unusual discharge or pain outside menstruation. These details help distinguish period-related cramping from inflammation or a condition affecting the uterus, ovaries or surrounding tissue.
Planning your next appointment
Bring a record of pain, bleeding, medicines tried and missed activities. A clinician can decide whether self-care is enough, whether an ultrasound is useful, or whether referral to gynaecology is appropriate. Treatment can be reviewed if symptoms persist or your family-planning needs change.

Questions people ask
How much period pain is too much?
Pain that limits daily activities, worsens, changes pattern or persists despite treatment should be assessed.
Can painful periods be caused by another condition?
Yes. Conditions affecting the uterus, ovaries or pelvic tissues can present with painful periods, so associated symptoms matter.
