Strength training also carries a risk of aggravating an injury when technique, range or resistance is poorly matched. Physiotherapy may be ineffective if attendance is not paired with the recommended activity between visits, or if treatment proceeds without revisiting an uncertain diagnosis. Stop and seek clinical advice for new marked swelling, sudden weakness, severe pain, numbness that is spreading or a major loss of function.

When neither approach should start immediately

Urgent assessment is more appropriate after major trauma, when a limb looks deformed, when you cannot bear weight, or when weakness or numbness is rapidly progressing. Fever with a hot swollen joint, chest symptoms, or new bladder or bowel control problems also need prompt medical attention. If pain is unexplained or steadily worsening, diagnosis and medical review should come before a loading programme.

A clinician matches the approach to the suspected problem, irritability of symptoms, current strength, movement quality, work or sport demands and personal goals. They may start with gentle movement and education, then add strengthening once the response is clear. Agree how progress will be measured, such as better stair control, longer walking or improved lifting technique.
