At a glance
Physiotherapy assesses the whole movement problem, while strength training rehabilitation progressively rebuilds force, endurance and load tolerance.
Physiotherapy is a broad clinical service that may include examination, education, mobility work, manual techniques and exercise. Strength training rehabilitation is a structured loading approach focused on restoring the ability of muscles and supporting tissues to produce and tolerate force. Strength work may be one part of physiotherapy, or it may be prescribed as a more focused rehabilitation pathway.
When physiotherapy may be the better starting point
Choose a physiotherapy assessment when the diagnosis is uncertain, symptoms involve more than weakness, or pain is limiting basic movement. It can help identify whether stiffness, balance, nerve sensitivity, technique or activity demands are contributing. A therapist can then select suitable exercises and other measures, explain how much activity is reasonable, and adapt the programme if symptoms change. This broader view is useful after an injury, operation or period of reduced mobility.
When strength training rehabilitation may fit
A focused strength rehabilitation plan may fit when the main limitation is reduced force or endurance and the involved area can tolerate progressive loading. It can be useful for rebuilding capacity for stairs, lifting, running, work tasks or sport. The programme should begin at a manageable level and progress through resistance, repetitions, movement range or task complexity. Good technique and recovery time matter as much as the exercise itself.
Risks and practical limits
Any rehabilitation exercise can cause temporary soreness, fatigue or an increase in symptoms if the load rises too quickly. Strength training also carries a risk of aggravating an injury when technique, range or resistance is poorly matched. Physiotherapy may be ineffective if attendance is not paired with the recommended activity between visits, or if treatment proceeds without revisiting an uncertain diagnosis. Stop and seek clinical advice for new marked swelling, sudden weakness, severe pain, numbness that is spreading or a major loss of function.
When neither approach should start immediately
Urgent assessment is more appropriate after major trauma, when a limb looks deformed, when you cannot bear weight, or when weakness or numbness is rapidly progressing. Fever with a hot swollen joint, chest symptoms, or new bladder or bowel control problems also need prompt medical attention. If pain is unexplained or steadily worsening, diagnosis and medical review should come before a loading programme.
How the plan is selected
A clinician matches the approach to the suspected problem, irritability of symptoms, current strength, movement quality, work or sport demands and personal goals. They may start with gentle movement and education, then add strengthening once the response is clear. Agree how progress will be measured, such as better stair control, longer walking or improved lifting technique. The plan should specify the starting load, warning signs, recovery days and when it will be reviewed.
Questions people ask
Is strength training part of physiotherapy?
Often it can be. Physiotherapy may include progressive strengthening alongside mobility, education, movement retraining and other care.
Can I start strengthening while I still have pain?
Sometimes, if the exercise and load are appropriate for the diagnosis and symptoms. A clinician can set boundaries and adjust the programme when the response is not suitable.
