White spots on the big toenail: what they may mean

Short answer
White spots on a big toenail are often caused by minor nail trauma, but persistent or changing marks should be checked.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.
At a glance
- Common clue
- The mark travels outward as the nail grows.
- Avoid
- Picking, deep filing and tight footwear.
What the mark may mean
A bump, tight footwear, nail biting or pressure during a pedicure can disturb the nail as it forms. The mark then moves toward the tip as the nail grows. A superficial white patch may also reflect dryness or separation between nail layers. Fungal nail problems more often cause yellowing, thickening, crumbling or debris, although appearances can overlap.
What to do now
Leave the spot intact and avoid scraping, filing deeply or covering it with harsh products. Wear shoes with room around the toes, keep the nail trimmed straight across, and dry the feet carefully. Take a clear photo and note whether the mark moves with growth. A clinician can examine the nail and, when needed, test material from it before choosing treatment.
When to seek care
Arrange an appointment if the mark does not move with nail growth, spreads, returns repeatedly, or comes with pain, swelling, redness, drainage, thickening or lifting. Seek prompt care for a dark streak, bleeding, a sore that does not heal, or a rapidly changing nail. People with diabetes, reduced circulation or reduced immunity should have new nail changes assessed sooner.
Questions for your doctor
Ask whether the appearance fits trauma, infection or another nail condition; whether a nail sample is useful; which products to stop; and how long to watch the change before review.

Questions people ask
Can a white spot on the big toenail be from an injury?
Yes. Minor pressure or impact can affect the forming nail and leave a mark that gradually moves toward the tip.
Should I remove a white spot myself?
No. Scraping or cutting into the nail can cause further injury and make assessment harder.
