White spots on the big toenail: what they may mean

A bump, tight footwear, nail biting or pressure during a pedicure can disturb the nail as it forms. The mark then moves toward the tip as the nail grows. A superficial white patch may also reflect dryness or separation between nail layers. Fungal nail problems more often cause yellowing, thickening, crumbling or debris, although appearances can overlap.

Leave the spot intact and avoid scraping, filing deeply or covering it with harsh products. Wear shoes with room around the toes, keep the nail trimmed straight across, and dry the feet carefully. Take a clear photo and note whether the mark moves with growth. A clinician can examine the nail and, when needed, test material from it before choosing treatment.

Arrange an appointment if the mark does not move with nail growth, spreads, returns repeatedly, or comes with pain, swelling, redness, drainage, thickening or lifting. Seek prompt care for a dark streak, bleeding, a sore that does not heal, or a rapidly changing nail. People with diabetes, reduced circulation or reduced immunity should have new nail changes assessed sooner.

Ask whether the appearance fits trauma, infection or another nail condition; whether a nail sample is useful; which products to stop; and how long to watch the change before review.
