Vomiting before a seizure: what to know

The timing matters. Nausea or vomiting can occur as an early warning for some people who have focal seizures, while infection, migraine, food poisoning, or gastroenteritis can cause vomiting and trigger a seizure through illness or dehydration. Meningitis is a serious possibility when vomiting occurs with fever, severe headache, a stiff neck, confusion, or unusual sleepiness. Gallstones and pancreatitis can cause marked abdominal pain alongside vomiting. In children, rotavirus infection may cause repeated vomiting and fluid loss.

Vomiting does not by itself identify the cause of a seizure. Record what happened before, during, and after the event, including awareness, movements, fever, headache, abdominal pain, recent illness, and medicines.

During a seizure, move hard or sharp objects away, cushion the head, loosen tight clothing, and turn the person onto their side when practical. Do not put food, drink, or anything in the mouth, and do not try to restrain them. Once fully alert, offer small sips of an oral rehydration drink if swallowing is normal. Arrange medical review after a first seizure, an unfamiliar seizure, or vomiting that continues.

Call emergency services if the seizure lasts longer than usual, repeats without recovery, causes breathing difficulty or injury, or the person remains confused or cannot be woken. Seek emergency help for vomiting with a severe headache, stiff neck, high fever, severe abdominal pain, a new rash, fainting, or inability to keep fluids down. A first seizure always warrants urgent assessment, even if the person seems better afterward.

Ask whether the episode could have been a seizure warning, whether tests are needed for infection or dehydration, and how to respond if vomiting or another seizure occurs. Share a timeline, witness account, video if safely recorded, and a list of medicines and recent illnesses.
