The short answer

Paleness can be easier to see on the face, lips, inside the mouth, palms, or the lining of the lower eyelid. Compare the area with its usual colour and consider whether the change appeared suddenly or gradually.

What very pale skin can mean

Some people naturally have light skin, and cold temperatures, fear, pain, or a temporary drop in blood flow can make anyone look paler. When the change continues, possible explanations include anaemia, which can reduce the amount of oxygen carried in the blood, or circulation problems that limit blood reaching the skin.

Iron deficiency anaemia may occur alongside tiredness, shortness of breath, headaches, dizziness, or a fast heartbeat. Vitamin B12 deficiency can also cause paleness and may be accompanied by numbness, pins and needles, balance problems, or a sore tongue. Inherited blood conditions such as thalassaemia or sickle cell disease can affect blood cells and may have additional symptoms. Pale skin alone cannot identify the cause.

What to do next

If the paleness is new, lasts beyond a brief trigger such as cold, or keeps returning, arrange a primary-care appointment. A clinician may ask about diet, menstrual or other bleeding, bowel changes, medicines, family history, and symptoms such as fatigue or breathlessness. They may examine your skin and eyes and suggest blood tests to check blood cells, iron stores, vitamin levels, and other relevant causes.

Do not start iron tablets simply because your skin looks pale. Iron supplementation is useful for confirmed or strongly suspected iron deficiency, but unnecessary iron can cause side effects and may be unsuitable for some conditions. Eat regular varied meals while arranging assessment, and record when the colour change occurs and any accompanying symptoms.

When to seek care urgently

Seek emergency help for sudden marked paleness with fainting, confusion, severe weakness, chest pain, severe difficulty breathing, cold clammy skin, or signs of major bleeding such as vomiting blood, black stools, or bleeding that will not stop. These symptoms can signal a serious problem with circulation, oxygen delivery, or blood loss.

Get prompt medical advice if paleness comes with yellowing of the eyes, dark urine, a high fever, unexplained weight loss, repeated infections, or worsening tiredness. A baby, child, pregnant person, or older adult with a noticeable new change should also be assessed without delay.
