What pale skin can look like

Pale skin can be noticed on the face, lips, gums, palms or nail beds and deserves assessment when it is new, persistent or accompanied by concerning symptoms.

Pallor is a lighter-than-usual appearance rather than a single shade. It may be easier to notice in the lining of the eyelids, inside the mouth, palms, nail beds or a familiar area of skin. Some people also feel tired, breathless on exertion, light-headed, cold, or aware of a fast heartbeat.

When to seek urgent help

Seek urgent medical help for sudden pallor with fainting, confusion, chest pain, severe shortness of breath, heavy bleeding, black stools, vomiting blood or a severe allergic reaction. These features can signal a problem needing prompt assessment.

Arrange a medical review soon if the change persists, is affecting daily activities, or comes with fatigue, dizziness, reduced exercise tolerance, yellowing of the eyes, unexplained bruising, fever or weight change.

Possible causes

How clinicians assess pallor

A clinician will ask when the colour change began, whether it fluctuates, and about diet, menstrual or other bleeding, medicines, family history and recent illness. Examination may include the skin, eyes, mouth, pulse, blood pressure and heart and lung sounds.

Blood tests may check the blood count, red-cell appearance, iron stores, vitamin B12 and other results selected from your history. Further tests depend on the findings, such as looking for a source of blood loss or checking for inherited blood conditions.

Treatment depends on the cause

Care targets the underlying reason for pallor. If testing identifies iron deficiency, a clinician may recommend dietary changes and iron supplementation, while also investigating why iron is low. Vitamin B12 deficiency, blood loss and inherited blood disorders need different plans.

Do not start high-dose iron solely because skin looks pale. Iron may not address other causes and can complicate interpretation of tests. Bring any supplements and medicines to the appointment.

Reducing avoidable contributors

A varied diet that includes iron-containing foods, vitamin B12 sources where appropriate, and regular meals supports blood cell production. If you follow a restricted diet, are pregnant, have heavy periods or have digestive symptoms, ask a clinician whether targeted testing or advice is suitable.
