What should you know about tophaceous gout?

How the structures involved in Gout differ from normal

Short answer

Tophaceous gout is longstanding gout in which urate crystals form firm deposits called tophi in and around joints.

This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.

At a glance

Deposit
A firm collection of urate crystals
Common sites
Joints and nearby tissues, including toes, fingers, elbows, and ankles
Key concern
Pain, restricted movement, or skin breakdown

How tophi affect the body

Tophi can develop around the toes, fingers, hands, elbows, ankles, knees, or ears. They may feel like hard lumps under the skin and can limit joint movement, press on nearby tissue, or become tender during a gout flare. The deposits may irritate the joint and contribute to ongoing joint pain or joint swelling. Tophaceous gout is a manifestation of gout, not a separate type of cancer. Chronic kidney disease and obesity can be relevant health factors, but a lump or swollen joint can have many causes and needs clinical assessment.

Steps that support care

Book an appointment with a clinician experienced in gout, particularly if a firm lump is growing, draining, or restricting movement. Assessment may include an examination, blood testing, imaging, or sampling material from a deposit when the diagnosis is uncertain. Take prescribed urate-lowering treatment consistently and do not change it during a flare without advice. Protect tender joints, use comfortable footwear, and discuss kidney health, alcohol, medicines, weight, and hydration rather than starting a restrictive diet alone.

When to seek care

Seek prompt medical care for a suddenly hot, very painful, swollen joint, especially with fever or feeling unwell, because infection can resemble a gout flare. Get urgent help for a wound over a tophus, pus, spreading redness, severe loss of movement, or new weakness or numbness. These warning signs should not be explained away by a known gout diagnosis.

Questions for your doctor

Ask whether the lump is a tophus, how gout control will be monitored, whether kidney function affects treatment, and how an affected joint can be protected. Discuss flare planning and whether a deposit needs specialist treatment because it interferes with movement or skin integrity.

Questions people ask

Are tophi always painful?

No. Some are painless at first, but they can later restrict movement, irritate tissue, or become painful during a flare.

Can a swollen knee be a tophus?

A tophus can affect the knee area, but knee swelling has other possible causes and should be examined.
