What should you know about surgical excision tattoo removal?

A dermatologist or surgeon marks the tattooed section, numbs the skin, removes the selected tissue, and brings the wound edges together with sutures. This approach is generally considered for a small tattoo or a limited part of a larger design where the skin can be closed without excessive tension. Unlike laser treatment, it does not break ink into smaller particles; the inked skin is removed with the specimen. The trade-off is a scar whose length may exceed the tattoo’s width, and the final appearance can change as the scar matures.

During consultation, discuss the tattoo’s size, location, colour, age, previous treatments, and your tendency to form raised or dark scars. Follow wound-care instructions closely: keep the dressing as directed, clean the site gently, avoid picking at crusts, and limit stretching or friction across the closure. Attend suture removal or wound checks when scheduled. Protect healed skin from strong sun because ultraviolet exposure can make a new scar more noticeable. If the tattoo is large, staged removal or another method may be discussed.

Contact the treating clinic promptly if pain, warmth, swelling, redness, or drainage is increasing rather than settling, or if the wound edges separate. Seek urgent medical help for heavy bleeding, rapidly spreading swelling, fever with feeling very unwell, or breathing difficulty. A scar that becomes increasingly raised, itchy, or tender also deserves review because treatment may help its symptoms.

Ask whether the whole tattoo or only a segment can be removed safely, where the scar will lie, how wound closure will be managed, and whether your skin history suggests a raised or pigmented scar. Ask what activities to avoid, when sutures come out, and what alternatives may suit you.
