Super nits: what they are and what to do

Super nits are head-lice eggs or lice in an infestation that may be less responsive to a particular treatment, so confirmation of live lice and tailored advice are important. The phrase does not describe a new species or unusually dangerous insect. It is commonly used when lice persist after a treatment has been used correctly, or when resistance to an ingredient is suspected.

A nit is an egg attached firmly to a hair close to the scalp. Empty shells and old eggs can remain after the infestation has cleared, so their presence alone does not prove that treatment has failed. Live crawling lice are the clearest sign of an active infestation. Persistent itching can also reflect irritation from scratching or a reaction that continues after the lice are gone; an itchy scalp is not, by itself, proof of resistant lice.

Treatment may appear unsuccessful if the product was not applied to every area, the instructions were not followed, reinfestation occurred from a close contact, or eggs hatched after the first application. Different products work in different ways, and some lice may be less affected by a specific ingredient.

Check the hair in bright light, especially behind the ears and at the nape, using a fine-toothed detection comb on wet, conditioned hair. Separate confirmed live lice from attached eggs, dandruff, and product residue. Follow the chosen treatment directions exactly, including any repeat application the product specifies, and avoid combining products unless a clinician or pharmacist advises it.

Check household and close contacts for live lice and treat people who have an active infestation according to local advice. Wash or isolate recently used hair items, but avoid excessive cleaning of the home; head lice mainly spread through close hair-to-hair contact. If live lice remain after correct use, arrange a review for an alternative approach.

Arrange a GP or pharmacist assessment when live lice are still present after following the product instructions, when repeated treatment has irritated the scalp, or when you are unsure whether what you see is a nit. Seek prompt medical advice for broken skin with spreading redness, warmth, swelling, pus, or worsening pain, because scratching can allow a secondary skin infection. Children, pregnant people, and anyone with a scalp condition should check that a treatment is suitable before using it.

Ask whether live lice are present, whether the previous product was used in the right way, and whether another ingredient or combing plan is appropriate. Tell the clinician when treatment was applied, which product was used, whether close contacts were checked, and whether the scalp is sore or showing signs of infection.
