Severe itchy scalp: what to know

Dandruff and seborrheic dermatitis may produce flaky scale, itch, and greasy or irritated patches around the scalp, hairline, ears, or eyebrows. Scalp psoriasis can form thicker, sharply defined scale that extends beyond the hairline. Contact dermatitis may begin after a new dye, shampoo, oil, or styling product. Folliculitis causes tender or itchy bumps around hairs. Head lice may cause persistent itch and visible nits attached close to hair shafts; itching can continue after exposure. A clinician can distinguish these patterns and check for infection or hair loss.

Stop any newly introduced scalp or hair product and rinse thoroughly if it caused burning or swelling. Avoid scratching, picking scale, or sharing combs, brushes, hats, and pillows. Wash gently and use a plain conditioner on hair lengths if needed. A medicated shampoo may help some scalp conditions, while head-lice treatment is used only when lice or nits are confirmed or a clinician advises it. Follow product directions carefully and avoid combining active treatments without guidance.

Arrange a dermatology or primary-care review if itch persists, causes sleep loss, returns often, or comes with thick scale, bleeding, crusting, pus, or patchy hair loss. Seek prompt care for painful swelling, fever, rapidly spreading redness, or facial swelling. Get urgent help for breathing difficulty after a hair product. If you find live lice, ask about treatment and household measures rather than treating repeatedly without confirmation.
