What should you know about retrocardiac pneumonia?

Retrocardiac pneumonia is a lung infection behind the heart that may need targeted imaging and medical treatment. The location can make signs less obvious on a standard front chest X-ray, so clinicians may consider your symptoms, examination and other views or scans together.

Retrocardiac means behind the heart. Pneumonia in this area can cause cough, fever, chills, phlegm, chest discomfort or breathlessness, although symptoms vary. A normal-looking initial image does not always settle the question when symptoms and examination suggest infection. Causes can include viruses such as influenza or COVID-19 and bacteria; treatment depends on the suspected cause and how unwell you are.

Arrange a clinical assessment if you have a persistent cough, fever, worsening tiredness, chest pain or shortness of breath. A clinician may check your breathing, oxygen level and temperature, then decide whether you need a chest X-ray, repeat imaging or another test. Take prescribed medicines exactly as directed, rest, drink fluids if you can, and avoid smoking. Do not start leftover antibiotics because they may not treat the cause.

Seek emergency help for severe difficulty breathing, blue or grey lips, new confusion, fainting, severe chest pain, or an inability to keep fluids down. Prompt assessment is also needed if symptoms rapidly worsen, you become unusually drowsy, or you have a serious long-term condition or weakened immune system. A spreading infection can become sepsis and needs urgent treatment.
