Allergy provocation test: what to know

A provocation test may be considered when the history, skin testing or blood testing does not clearly show whether an allergen causes symptoms. In a clinic, the suspected substance is given in carefully planned amounts while staff monitor breathing, skin, blood pressure and other symptoms. The test may involve food, medicine or another exposure, depending on the specialist assessment. It is different from trying the suspected trigger at home. A positive response can support an allergy diagnosis, while a negative supervised result may help guide safe decisions; the result must be interpreted with the full history.

Follow the clinic’s instructions about antihistamines and other medicines, because some can affect testing. Tell the team about asthma, recent illness, pregnancy, previous severe reactions and every medicine or supplement you use. Bring details of the original reaction, including what was eaten or taken, timing and symptoms. Do not deliberately expose yourself before the appointment. The test can help clarify suspected food allergy, pet allergy, house dust mite allergy or another allergic condition, but the specialist decides whether it is suitable.

During or after any exposure, seek emergency help for throat or tongue swelling, breathing difficulty, wheezing, faintness, confusion, widespread hives with vomiting, or rapidly worsening symptoms. A provocation test must never be carried out without the prescribed clinical supervision and emergency equipment. If you have asthma, ask how symptoms will be monitored because uncontrolled asthma can increase concern during testing. Follow the discharge plan and report delayed symptoms to the clinic.
