Post partum vaginal itch: what to know
Short answer
Postpartum vaginal itching may come from healing, dryness, irritation, thrush or a skin condition, so persistent or severe itch needs an examination.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.
At a glance
- Possible triggers
- Healing tissue, pads, soaps, moisture, dryness and thrush can all cause itch.
- Avoid
- Scented products, douching, scratching and unprescribed treatments.
- Get checked
- Persistent itch, sores, unusual discharge or worsening pain needs assessment.
Why itching can happen
After birth, stitches, tears, swelling, lochia, pads, soaps or moisture can irritate the vulval skin. Hormonal changes may cause dryness, especially while breastfeeding. Thrush can cause intense itch with soreness or a thick discharge, but similar symptoms can occur with other infections or skin problems.
A clinician can inspect the vulva and vagina and test discharge when needed. Lichen Sclerosus can affect vulval skin and needs specific assessment. Genitourinary Syndrome of Menopause is usually linked with later hormonal change, but its symptoms can resemble postpartum dryness.
Comfort and practical care
Rinse gently with water, pat dry and avoid scented washes, sprays, douches and scrubbing. Change damp pads or underwear, choose loose breathable clothing and avoid scratching. Do not insert creams or use leftover antibiotics without advice, particularly if you are breastfeeding or have healing stitches.
If examination confirms thrush, your clinician may recommend Antifungal Treatment for Thrush. If the problem is an inflammatory vulval skin condition, Topical Steroid Treatment for Vulval Skin may be appropriate; the right product and duration depend on the diagnosis.
When to seek care
Arrange prompt assessment if itching is severe, persistent, spreading or accompanied by unusual discharge, strong odour, sores, blisters, bleeding, fever, increasing pain or difficulty passing urine. Seek urgent care for rapidly worsening swelling, severe pain, heavy bleeding or feeling faint. Contact your maternity team if a wound or stitches look open or infected.
Questions for your doctor
Could this be thrush, irritation, an infection or a skin condition? Do I need a swab or examination? Which products are safe while breastfeeding? How should I care for stitches or a tear? What changes mean I should return urgently?

Questions people ask
Is postpartum vaginal itching always thrush?
No. Irritation, healing, dryness and vulval skin conditions can feel similar, so an examination may be needed before treatment.
Can I use an over-the-counter cream after birth?
Ask a clinician or pharmacist first, especially if you are breastfeeding, have stitches, or have pain, sores or unusual discharge.
