Post-nasal drip and severe cough: what to know

Mucus from the nose or sinuses may slide down the back of the throat, causing repeated throat-clearing, a tickle, hoarseness, or coughing that is worse when lying down. A forceful cough can then make the throat more sensitive, creating a cycle of irritation.

Nasal congestion, sneezing, itchy eyes, or clear mucus can fit with allergic rhinitis. A blocked or runny nose with a sore throat and fatigue can occur with a common cold. Thick mucus, facial pressure, or a cough after a respiratory infection may need an examination to distinguish nasal drainage from a lower-airway problem.

A cough can also come from acid reflux, asthma, or bronchitis, even when post-nasal drip is present. The pattern matters: wheezing, chest tightness, breathlessness, fever, or coughing up blood should not be explained away as mucus alone. A cough that continues or repeatedly returns deserves a clinical review.

Drink fluids regularly, use a saline nasal spray or rinse as directed, and consider a humidified room if dry air worsens the irritation. Elevating your head while sleeping may reduce drainage into the throat. Avoid smoke, vaping, strong fragrances, and other triggers that make coughing worse.

Do not start antibiotics or combine cough and cold products without checking the ingredients and whether they suit you. A pharmacist or clinician can advise about treatment for allergies, nasal inflammation, reflux, or asthma. Seek assessment if self-care is not helping, the cough is worsening, or it is disrupting sleep, eating, or breathing.

Get urgent medical help for severe difficulty breathing, blue or grey lips, new confusion, fainting, chest pain, or coughing up blood. Prompt assessment is also appropriate for a high or persistent fever, marked weakness, dehydration, or a cough that is rapidly getting worse.

Arrange a routine appointment if the cough lasts beyond the expected course of a cold, keeps returning, or comes with wheezing, nighttime symptoms, weight loss, or recurrent chest infections. Babies, older adults, pregnant people, and anyone with lung or immune problems may need earlier advice.

Ask whether the cough pattern fits nasal drainage, reflux, asthma, or an infection, and whether your nose, throat, chest, or breathing needs examination. Mention when it began, whether it is dry or produces mucus, what happens at night, possible allergy or smoke exposures, and every medicine you take.
