Pneumonia without a cough

A cough develops when irritated airways need to clear mucus, but infection deeper in the lungs may not trigger much coughing. Older adults and people with reduced immunity may show less typical signs, such as sudden weakness, sleepiness or confusion. Other possible features include faster breathing, breathlessness during ordinary activity, chest pain when breathing, chills, fever, poor appetite or low energy. Pneumonia linked with COVID-19 or influenza can also vary from person to person. The absence of cough does not confirm or exclude the condition.

Seek a clinical assessment if breathing is more difficult, you have chest discomfort, feel markedly unwell, or symptoms are progressing without a clear explanation. A clinician may examine your chest and decide whether oxygen measurement, imaging or other tests are appropriate. Rest, drink fluids if safe, avoid smoke and follow any treatment instructions. Note changes in breathing, temperature, alertness and ability to eat or drink. Do not wait for a cough to appear before asking for help.

Call emergency services for severe breathlessness, blue or grey lips, fainting, new confusion, chest pain or difficulty staying awake. Obtain prompt medical advice if breathing worsens, fever or chills appear, or you become too weak to drink. A serious infection can worsen even when coughing is minimal.
