Pneumonia with a normal chest X-ray

A chest X-ray looks for visible changes in the lungs, but an infection may not show clearly at the beginning. The image can also be harder to interpret when symptoms are mild, the affected area is small, or another condition changes the appearance of the lungs. A clinician considers fever, cough, breathing difficulty, chest discomfort, oxygen level and examination findings alongside the image. Other explanations, including bronchiectasis, tuberculosis or a persistent cough, may need consideration when symptoms do not fit the X-ray result.

Arrange a clinical review if symptoms continue, worsen or are limiting normal activity. Take prescribed medicines exactly as directed and do not start, stop or share antibiotics without advice. Your clinician may recommend observation, a repeat assessment, another test or treatment based on the complete picture. Keep a record of fever, breathlessness, chest pain and changes in cough so the review is more useful.

Seek urgent medical help for severe difficulty breathing, blue or grey lips, new confusion, fainting, severe chest pain, or an inability to keep fluids down. Prompt assessment is also sensible if breathing becomes rapidly harder or you feel markedly weaker. A normal image should not delay care when red-flag symptoms are present.
