Pneumonia in older adults: symptoms and care

Pneumonia affects the air sacs in the lungs and can make oxygen movement harder. An older adult may have cough, phlegm, chills, fever, breathlessness, chest discomfort, or unusual tiredness. Instead, the first change may be confusion, sleepiness, reduced appetite, a fall, or difficulty managing usual activities. Existing heart or lung disease can make symptoms harder to interpret. A clinician may assess oxygen, breathing, hydration, medicines, and chest findings, sometimes using imaging or other tests.

Arrange medical assessment promptly when pneumonia is suspected. Treatment may involve antibiotics for a bacterial cause, fluids, oxygen, or hospital observation; the choice depends on examination findings and other health conditions. Keep a clear record of medicines and allergies. Encourage rest, fluids if swallowing is safe, and gentle nutrition. Help the person sit upright if that eases breathing, and watch alertness, urine output, and the ability to drink. Never change prescribed medicines without medical advice.

Call emergency services for severe breathlessness, blue or grey lips, new severe confusion, fainting, chest pain, inability to stay awake, or inability to drink. Urgent assessment is also needed when breathing becomes noticeably harder, weakness is sudden, or symptoms worsen despite treatment. Do not leave a confused or breathless person alone while arranging help.

Ask which findings support pneumonia, whether a chest image or oxygen check is needed, which treatment is planned, how existing conditions affect that plan, and what improvement should look like. Confirm the follow-up date and the exact signs that should trigger a return visit.
