Perimenopause bleeding during intercourse: what to know

Hormone changes during the menopause transition may make vaginal tissue thinner, drier, or more easily irritated. Friction can then cause light spotting, soreness, or a small amount of blood after sex. Bleeding is not something to diagnose from symptoms alone.

Perimenopause can bring changing hormone levels and irregular periods, so blood noticed after sex may sometimes be menstrual bleeding that happened to start around intercourse. Dryness and inflammation can also cause contact bleeding. Other possibilities include an infection, a cervical or vaginal growth, or changes that need examination. A clinician may ask about the timing, amount, pain, discharge, contraception, and whether pregnancy is possible.

Keep a note of whether the blood appears during sex, immediately afterward, or at another time, and whether it happens again. This detail can help separate bleeding related to friction from bleeding between periods.

Arrange an appointment with a doctor or gynaecology clinician for new, repeated, or unexplained bleeding during or after sex. Until you are assessed, pause penetration if it causes pain or bleeding, and consider a water-based lubricant when you resume if sex feels comfortable. Do not insert products or use treatments for infection unless a clinician or pharmacist has advised you.

Tell the clinician about any dryness, burning, unusual discharge, pelvic pain, bleeding between periods, or changes in your usual cycle. Depending on your history, the assessment may include an examination and tests for infection or other causes.

Seek urgent medical help if bleeding is heavy or does not settle, or if it comes with severe pelvic or abdominal pain, faintness, marked weakness, fever, or feeling very unwell. Seek prompt assessment as well if pregnancy is possible and bleeding is accompanied by pain. These features can need quicker evaluation than routine perimenopause symptoms.

Bleeding after sex that keeps returning, occurs after periods have stopped, or is associated with a new lump, persistent pelvic pain, or unusual discharge should not be put down to perimenopause without an examination.
