What it means
The pars interarticularis connects the upper and lower joints at the back of each vertebra. Repeated bending backwards and rotation can place repeated force through this small area. The defect is often in the lower spine and may cause one-sided or central lower-back pain, especially with sport, standing, or extension. Some people have no symptoms. A defect may occur on one side or both sides, and it does not automatically mean that the vertebra has slipped.
What to do
Assessment usually combines your symptoms and physical examination with imaging chosen by a clinician. Until you have advice, reduce movements that reproduce the pain, particularly repeated back arching and forceful rotation. Maintain comfortable walking and ordinary activity where possible. Physiotherapy can teach neutral-spine control, trunk endurance, hip mobility, and a staged return to sport or work. Exercise therapy should be progressed gradually, with pain response and movement quality guiding each step. Surgery is considered only in selected situations when symptoms remain significant despite appropriate non-surgical care.
When to seek care
Book a clinical review if back pain persists, repeatedly returns with activity, or interferes with sleep, school, work, or sport. Seek urgent care for new weakness in a leg, numbness around the inner thighs or genitals, or difficulty controlling urine or stool. A new radiating pain pattern also deserves timely review because the clinician may need to check for nerve involvement.
