Why mottling appears
Mottling is a patchy or net-like pattern caused by uneven blood flow near the skin. It may be temporary after cold exposure and improve with gentle warming. Note whether the skin is cold, painful, swollen, numb or changing quickly. General paleness with tiredness or breathlessness can occur in anaemia, but anaemia does not explain every mottled pattern.
Mottling during a severe illness can accompany poor circulation, low body temperature or another serious problem. A pale, blue or grey colour around the lips, marked drowsiness or difficulty breathing is more concerning than a stable pattern that fades as the skin warms.
What to do now
Move to a warm environment, remove wet clothing and recheck the skin after gentle warming. Avoid very hot water or direct heat on numb skin. Photograph the pattern in good light if it comes and goes, and record triggers, duration and associated symptoms. Arrange medical review if mottling persists, recurs without cold exposure or appears with fever, pain, swelling, weakness or a new illness.

Seek emergency help for sudden mottling with trouble breathing, chest pain, fainting, confusion, severe weakness, blue or grey lips, a very cold limb or rapidly worsening illness. A leg or arm that is pale and mottled with severe pain, numbness or reduced movement may have an urgent circulation problem. Do not wait for warming to resolve these signs.
