Pain During Ectopic Pregnancy: What to Know

An ectopic pregnancy develops outside the main cavity of the womb, often in a fallopian tube. Pain may be low in the abdomen or pelvis, commonly more noticeable on one side. It can begin as a persistent ache or become sharp. Vaginal bleeding, shoulder-tip pain, rectal pressure, or pain when passing urine or stool can also occur. Symptoms may change quickly, and early symptoms can resemble other pelvic problems, including pelvic inflammatory disease. Pain alone cannot confirm the diagnosis.

If pregnancy is possible and you have pelvic pain, contact an urgent medical service for advice rather than waiting for symptoms to settle. A clinician may arrange pregnancy-hormone testing and a transvaginal ultrasound, sometimes with repeat assessment when the findings are not yet clear. Do not drive yourself if you feel faint or unwell. Keep a note of bleeding, pain location, and any change in symptoms.

Call emergency services or go to an emergency department for sudden severe pain, fainting, collapse, shoulder-tip pain, heavy bleeding, severe weakness, or a fast deterioration. These signs can indicate internal bleeding from a ruptured ectopic pregnancy. Treatment may involve medicine or surgery, depending on the pregnancy location, symptoms, test results, and your overall condition. A salpingectomy is one surgical option when a damaged fallopian tube must be removed.
