What a miscarriage may look like
Updated

Short answer
A miscarriage may look like bleeding that becomes heavier, lower abdominal cramps, clots, or greyish tissue passing from the vagina. Some people have light bleeding or no obvious tissue. Bleeding in pregnancy does not always mean miscarriage, so symptoms alone cannot confirm what is happening.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.
At a glance
- Possible signs
- Bleeding, cramps, clots, or tissue may occur.
- Diagnosis
- Symptoms alone cannot confirm miscarriage.
How symptoms can vary
The amount and pattern of bleeding can differ according to how far the pregnancy has developed and whether tissue has passed. Cramping may come in waves as the uterus contracts. A clinician may ask about dates, bleeding, pain, and pregnancy symptoms, then use examination, pregnancy testing, or a Transvaginal Ultrasound to clarify the diagnosis.
What to do if you notice these signs
Use pads rather than tampons while bleeding so you can observe the flow, and note when symptoms start and whether pain is worsening. If tissue passes, you may place it in a clean container only if a clinician asks for it; do not feel pressured to do so. Arrange medical advice promptly. Depending on the assessment, Medical Management of Miscarriage or Surgical Management of Miscarriage may be options.
When to seek urgent help
Seek emergency care for heavy bleeding, severe or one-sided pain, fainting, shoulder-tip pain, fever, or feeling very weak or unwell. These symptoms can require immediate treatment, including assessment for ectopic pregnancy or infection.
Questions for your doctor
Could this bleeding have another cause? Do I need an ultrasound or repeat pregnancy test? Has all tissue passed? What pain relief is suitable? Which changes mean I should return urgently?

Questions people ask
Can a miscarriage happen without visible tissue?
Yes. Tissue may be too small to notice, or the diagnosis may require an examination or ultrasound.
Is spotting always a miscarriage?
No. Spotting can have several causes, but bleeding in pregnancy should be discussed with a clinician.
