Short answer

Hypothalamic anovulation is a pause in ovulation caused by disrupted hormone signalling from the hypothalamus to the ovaries.

This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.

The short answer

Hypothalamic anovulation is a pause in ovulation caused by disrupted hormone signalling from the hypothalamus to the ovaries. The pattern may follow low energy availability, intense exercise, emotional strain, significant weight change, or another health problem. Without ovulation, periods may become irregular or stop, and pregnancy may be harder to achieve.

What it means

The hypothalamus helps coordinate the signals that prompt the pituitary gland and ovaries to prepare an egg. When the body senses that conditions are not suitable for reproduction, these pulses can slow. This is different from polycystic ovary syndrome and premature ovarian insufficiency, although the symptoms can overlap. A clinician may ask about eating patterns, exercise, stress, medicines, weight changes, headaches, vision changes, and other periods of absent or irregular bleeding.

Assessment can include a pregnancy test, hormone testing, and sometimes thyroid or other checks. The aim is to identify the reason for the signal change, protect bone health when periods remain absent, and understand fertility goals.

What you can do next

Arrange a gynaecology or fertility assessment if cycles have changed or you are trying to conceive. Keep a record of bleeding, exercise, meals, weight changes, medicines, and symptoms. Treatment focuses on restoring adequate nutrition and recovery, reducing excessive training where relevant, and addressing stress or an underlying illness. If pregnancy is desired and ovulation does not return, a clinician may discuss ovulation induction after the cause has been assessed. Metformin or ovarian drilling are not general treatments for hypothalamic anovulation; they relate to selected cases of polycystic ovary syndrome.

When to seek care

Seek prompt medical advice for absent periods with a positive pregnancy test, severe pelvic pain, fainting, heavy bleeding, new severe headaches, or vision changes. Arrange routine care for repeated irregular periods, trouble getting pregnant, marked weight change, restrictive eating, or exercise that leaves you exhausted. These features can point to a need for broader hormonal, nutritional, or general health assessment.

Questions for your doctor

What could be interrupting my ovulation? Which hormone tests or other checks do I need? Should I change my exercise or nutrition plan? How will we protect bone health if periods remain absent? What options are appropriate if I want pregnancy now?

Questions people ask

Can hypothalamic anovulation affect fertility?

Yes. If ovulation is not occurring, an egg is not released for fertilisation. Fertility can improve when the underlying cause is addressed, and medical ovulation support may be considered.

Is hypothalamic anovulation the same as PCOS?

No. Both can involve irregular periods, but their hormone patterns and treatment plans differ. Testing and the wider history help distinguish them.
