What a hydrotherapy treadmill does

The treadmill sits inside a pool or tank. Water can support part of your body weight, making stepping easier when land walking is uncomfortable. The water also slows movement and provides resistance in more than one direction, so your muscles work without the same impact as ordinary walking. A physiotherapist may adjust water depth, belt speed and exercise time to match your balance, strength and symptoms.

It may be included in rehabilitation for joint pain or joint stiffness, including after an injury or operation. It is not a replacement for assessment, strengthening or home exercises. People with conditions such as Ankylosing Spondylitis, Fibromyalgia or Frozen Shoulder may have different goals, so the programme should be personalised.

How to prepare and participate

Wear suitable swim clothing and follow the facility's guidance about changing, footwear and hygiene. Tell the therapist about open wounds, skin problems, dizziness, breathing difficulties, continence concerns or fear of water before entering. Begin at a comfortable pace and use the handrails if instructed. Your therapist may combine treadmill walking with balance drills, gentle joint movements or land-based Exercise Therapy.

Expect to feel warm and mildly worked, but stop if you develop sharp pain, unusual weakness, light-headedness or breathlessness. Drink fluids as advised and allow time to dry and change safely after the session.

When to seek care

Seek urgent medical help for chest pain, severe breathing difficulty, fainting, sudden weakness or a serious fall. Contact your clinician promptly if a treated joint becomes increasingly hot, swollen or painful, or if you develop fever or drainage from a wound. These symptoms need assessment rather than another pool session.

Is an underwater treadmill the same as swimming?

No. You remain upright and practise stepping on a moving belt; swimming uses different positions and movement patterns.

Can it cure joint pain?
