How the pool helps movement

Water supports part of your body weight, which can reduce loading through painful joints and make walking, stretching or strengthening feel more manageable. Warm water may help muscles relax and can make stiff movement easier. The water also provides gentle resistance, so a slow arm or leg movement can become a controlled strengthening exercise without relying only on weights.

Hydrotherapy is not the same as swimming laps. A physiotherapist usually chooses movements for your symptoms, balance, strength and goals. Sessions may be useful for joint pain or joint stiffness and may form part of rehabilitation for conditions such as ankylosing spondylitis, fibromyalgia or frozen shoulder. The pool environment can also make it easier to practise a movement before progressing to land-based exercise therapy.

What to do before and during a session

Tell the treating team about a new illness, open wound, skin problem, bowel or bladder issue, dizziness, breathing difficulty, or a change in pain before entering the pool. Bring suitable swimwear, drinking water and any prescribed pool footwear or support. Follow instructions for entering and leaving the pool because wet surfaces can be slippery.

During treatment, work at the pace set by your physiotherapist rather than trying to copy another swimmer. Stop and report increasing pain, unusual shortness of breath, light-headedness, nausea or feeling unwell. The aim is controlled movement; soreness that continues or worsens after a session should be discussed before the next appointment.

When to seek medical advice

Ask a clinician for advice before hydrotherapy if you have a fever, an untreated wound, a spreading skin infection, uncontrolled symptoms, or difficulty keeping yourself safe in water. Contact your healthcare team promptly for chest pain, severe breathing trouble, fainting, new weakness, or a sudden major change in pain or function. These symptoms need assessment rather than an exercise session.
