Heart cough: what should you know?

Heart failure can make the heart less able to move blood forward. Pressure may then rise in the blood vessels of the lungs, allowing fluid to collect. The cough may be persistent, worse when lying flat, or accompanied by breathlessness, waking at night, reduced exercise tolerance, or swollen ankles. These features do not confirm a heart problem on their own. Acid reflux, asthma, allergies, a cold, bronchitis, and chronic cough are also possible explanations. A cough is a common presentation of many airway and digestive conditions, so the direction of the clue matters: the symptom does not identify one diagnosis.

A clinician may ask when the cough began, whether it produces mucus, what position worsens it, and whether your weight, breathing, or medicines have changed.

Record triggers, timing, mucus, breathlessness, sleep disruption, and any ankle swelling. Take prescribed medicines as directed and do not stop a heart or blood-pressure medicine without medical advice. Avoid smoke and notice whether lying flat changes symptoms. Arrange a GP assessment if the cough persists, limits activity, or occurs with a known heart condition. The appointment may include a chest and heart examination, a medication review, and tests chosen for your symptoms.

Seek emergency help for severe or rapidly worsening breathlessness, blue or grey lips, fainting, new confusion, chest pressure, or coughing up pink or bloody froth. Get prompt medical advice for a new cough with increasing swelling, sudden loss of exercise ability, or breathlessness that is worse when lying down.
