Hallux rigidus pads: what to know

Short answer
Hallux rigidus pads may reduce pressure around a stiff big-toe joint, but they do not restore joint motion or correct the underlying arthritis.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.
At a glance
- Purpose
- Cushioning or redistributing shoe pressure
- Limitation
- A pad cannot restore big-toe movement
- Fit check
- No rubbing, numbness or skin damage
What a pad can and cannot do
Hallux rigidus affects the joint where the big toe meets the foot. A pad can cushion a tender prominence or redistribute contact inside a shoe. The useful effect depends on its shape and exact position: pressure placed directly over a painful bump may aggravate symptoms, while a carefully fitted design may leave more room around the joint. Pads do not remove joint stiffness, and a bulky insert can make a narrow shoe worse. Using pads thoughtfully
Use a pad only with footwear that has enough length, width and upper-room for the toe. Stop using it if it causes rubbing, numbness, skin breakdown or greater pain. Test it during ordinary walking before a long outing. A clinician or podiatrist can assess whether a pad, shoe modification or [Custom Orthotics](/treatments/non-surgical-orthopedics/custom-orthotics/) is more suitable. Persistent joint pain may lead to a discussion of a [Cortisone Injection](/treatments/non-surgical-orthopedics/cortisone-injection/). In advanced cases, [Joint Fusion Surgery](/treatments/reconstructive-orthopedic-surgery/joint-fusion-surgery/) may be considered after examination.
When to seek care
Book an assessment when a pad no longer lets you walk comfortably, symptoms are progressing, or you are changing how you place your foot. Seek prompt care for a hot, red and rapidly swollen joint, fever, an open sore, new loss of sensation or inability to bear weight after an injury.
Questions for your doctor
Where should pressure be reduced on my foot? Is a pad appropriate for my shoe shape? Would an orthotic provide better support? How will we tell whether pain comes from the joint or irritated skin? What symptoms should prompt a review?

Questions people ask
Can pads cure hallux rigidus?
No. They may improve comfort by changing pressure, while the stiffness and joint changes remain.
Why does a pad make my toe hurt more?
It may be pressing on the joint, crowding the shoe or shifting pressure to a sensitive area; remove it and have the fit checked.
