What toe pain feels like

Pain may be throbbing, burning, tender to touch, stiff with movement, or sharp when the toe pushes off. You might notice swelling, warmth, redness, a changed toe shape, rubbing in shoes, or pain beneath the ball of the foot. One toe or several toes can be involved, and symptoms may be worse after walking, sport, or wearing a narrow shoe.

Is toe pain urgent?

Arrange prompt care after a crush injury, obvious deformity, inability to walk, rapidly increasing swelling, or a toe that becomes pale, blue, cold or numb. A hot, red, very tender joint with fever needs urgent assessment. If symptoms are milder, book a review when pain persists, returns frequently, or stops you wearing normal footwear.

Common causes of toe pain

The causes are listed alphabetically, not by likelihood. Bunions change the alignment around the big-toe joint and can create shoe pressure. Gout can cause sudden inflammatory pain and swelling in a joint. Hallux rigidus makes the big-toe joint stiff and painful during push-off. Hammer toe bends a smaller toe and may rub against footwear. A plantar plate tear affects a stabilising structure beneath a toe joint. Turf toe is a sprain of the big-toe joint caused by forceful upward bending.

How toe pain is diagnosed

A clinician asks about onset, footwear, activity, injury, previous episodes and the exact toe involved. They inspect alignment, skin, swelling and bruising, then test joint movement, tenderness, stability and walking. X-rays or other imaging may be considered for a fracture, joint change or deformity. If an acutely inflamed joint is suspected, the clinical history and examination guide the next tests and treatment.

Treatment depends on the cause

Care may include roomier footwear, activity changes, padding, a stiff-soled shoe, exercises or prescribed medication. Bunions and hammer toes may benefit from pressure reduction and alignment support; persistent structural problems can need specialist review. Joint inflammation needs cause-specific medical treatment. A plantar plate injury or turf toe may require protected loading followed by rehabilitation. Bunion surgery is one option considered when symptoms remain limiting despite non-surgical care.

Protecting your toes

Choose shoes with enough length and width for the toes to spread without rubbing. Increase running, jumping and court activity gradually, and replace worn footwear when it no longer supports your movement. Use technique and training drills that prepare the big toe for push-off. After an injury, return to impact only when walking and sport-specific movements are controlled and comfortable.

Symptoms that may occur with toe pain

Look for redness, warmth, swelling, bruising, stiffness, numbness, altered balance, a visible bend, or pain beneath the forefoot. These details help identify whether pressure, inflammation, joint restriction or a supporting-tissue injury is involved.

Choosing the next step

Note which toe hurts, whether shoes or push-off trigger symptoms, and whether the problem started suddenly. That information helps a clinician decide whether footwear changes, rehabilitation, imaging or specialist treatment is appropriate.

Questions people ask

Can shoes cause toe pain?

Yes. Narrow or short shoes can press on a deformity, restrict the big toe, or increase stress on tissues beneath the forefoot.

When should toe pain be checked?

Arrange an assessment for persistent, recurrent or limiting pain, and seek prompt care for severe injury, marked deformity, colour change, numbness or a hot swollen joint with fever.
