Groin Strain and Testicle Pain: What to Know

Short answer
Groin strain testicle pain can reflect nearby muscle or nerve irritation, but sudden or severe testicle pain needs urgent medical assessment.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.
At a glance
- Common trigger
- Running, kicking, sudden direction changes, or lifting can strain the inner-thigh muscles.
- Urgent warning
- Sudden or severe testicle pain requires urgent medical assessment.
- Rehabilitation
- Progressive movement and strengthening should match symptoms and clinical guidance.
The short answer
The inner-thigh adductor muscles attach near the pubic bone, close to structures that can make discomfort feel as though it is in the groin, scrotum, or lower abdomen. A strain is more suggestive when pain began after sprinting, kicking, changing direction, lifting, or another forceful movement and increases when the leg is squeezed inward.
What the pain may mean
A groin strain can cause local tenderness, pain with walking or running, and discomfort when the hip is moved or the thighs are brought together. Pain felt near a testicle is not proof that the testicle itself is injured, and testicle pain is not proof of a groin strain. Hernia, infection, urinary or kidney problems, and twisting of the testicle can also cause pain in this area. A clinician may examine the abdomen, groin, hips, and scrotum to separate these possibilities.
What to do now
Stop the activity that triggered the pain and use relative rest. For a recent muscle injury, a wrapped cold pack may ease discomfort for short periods; keep it off the skin. Supportive underwear can reduce movement-related scrotal discomfort. Do not stretch forcefully, massage a painful lump, or return to sprinting while walking remains painful. A physiotherapist can guide gradual mobility and strengthening through [Exercise Therapy](/treatments/physiotherapy-and-rehabilitation/exercise-therapy/) and, for athletes, a [Sports Physiotherapy](/treatments/physiotherapy-and-rehabilitation/sports-physiotherapy/) assessment.
When to seek care
Seek emergency care for sudden or severe testicle pain, a testicle that appears higher or sits differently, marked swelling, nausea or vomiting, fever with scrotal pain, or pain after a significant injury. These symptoms can signal a problem that cannot be safely diagnosed as a strain at home. Arrange a medical review for persistent pain, a groin or scrotal lump, urinary symptoms, discharge, or symptoms that are not settling. If recovery is progressing, a [Return-to-Sport Programme](/treatments/physiotherapy-and-rehabilitation/return-to-sport-programme/) can help test strength and movement before full activity.
Questions for your doctor
Ask: Could the pain be coming from the adductor muscles, hip, hernia, urinary tract, or testicle? What examination or scan is appropriate? Which activities should I pause, and what movement is safe? What warning signs mean I should return urgently?

Questions people ask
Can a groin strain cause pain near the testicle?
Yes. Irritation in nearby groin muscles, joints, or nerves can be felt toward the scrotum, although other causes of testicle pain must be considered.
Should I exercise with groin strain testicle pain?
Pause activities that reproduce the pain. Gentle movement may be suitable, but exercise progression should wait until urgent causes have been excluded and should not increase symptoms.
When is testicle pain an emergency?
Sudden or severe pain, swelling, a changed testicle position, nausea or vomiting, fever, or significant injury warrants emergency assessment.
