Why the symptoms can overlap

The groin, lower abdomen, hip and scrotal area share nearby nerves, so pain from an inner-thigh muscle may be felt higher or lower than the injured tissue. Even so, testicular pain is not a reliable way to identify a groin pull. Hip impingement, a hip labral tear and hip osteoarthritis can refer pain towards the groin, while other scrotal or abdominal problems require separate medical evaluation. Do not self-diagnose from the timing of pain after exercise.

What to do next

Stop strenuous activity and arrange a prompt clinical assessment if the testicle itself hurts, feels swollen or tender, or the discomfort is new and unexplained. Wear supportive clothing if it feels more comfortable, and avoid repeated pressing or checking. If a clinician confirms a muscle source, rehabilitation may later address adductor and hip strength. Sports Physiotherapy is appropriate only after urgent causes have been considered.

Urgent warning signs

Seek emergency care for sudden or severe testicular pain, a high-riding or unusually positioned testicle, rapid swelling, nausea or vomiting, fever, redness, faintness, or pain after significant injury. Prompt assessment matters even when pain began during sport. A new groin bulge, inability to pass urine, or rapidly worsening lower-abdominal pain also needs urgent attention. Do not wait for rest to clarify severe testicular symptoms.
