Gout or joint infection? Understanding the difference

Short answer
Gout is caused by urate crystals, not bacteria, but a gout flare and joint infection can look very similar.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.
At a glance
- Cause of gout
- Urate crystals inside a joint
- Cause of infection
- Germs entering a joint
Why the distinction matters
Gout produces inflammation when crystals activate the immune response inside a joint. A joint infection occurs when germs enter the joint and can worsen quickly. Both may cause pain, heat, redness, swelling, and difficulty moving. Fever may occur with infection, but its absence does not safely rule infection out. A clinician may need to examine the joint and test joint fluid for crystals and germs; blood tests or imaging may also help. Do not assume a familiar gout pattern is harmless if this episode feels different.
The [Gout](/conditions/gout/) page explains the underlying condition. Tell the clinician about [Chronic Kidney Disease](/conditions/chronic-kidney-disease/) or other conditions that affect treatment choices.
What to do with a hot swollen joint
Contact a healthcare professional promptly for a new hot, swollen joint, especially if pain is severe or movement is restricted. Keep the joint rested and avoid squeezing, massaging, or puncturing the skin. Take only medicines already recommended for you, and do not use leftover antibiotics. If you have a prescribed flare plan, ask whether it is appropriate while infection is being considered. Note when symptoms began, whether they are spreading, and any recent wound, injection, surgery, or illness.
Urgent warning signs
Seek emergency care for fever with confusion, faintness, rapid breathing, severe weakness, or rapidly spreading redness. Same-day assessment is needed for a hot swollen joint with chills, pus, a wound nearby, or pain that is escalating. Infection can threaten joint function and needs treatment chosen by a clinician.
Questions to ask
Ask whether joint fluid should be tested, how infection will be excluded, and which pain treatment is safe with your medicines. Ask when to return if symptoms worsen and how future [Joint Swelling](/symptoms/joint-swelling/) should be assessed. Mention [Obesity](/conditions/obesity/) if relevant to your care.

A hot swollen joint should be assessed promptly, particularly with fever, chills, a wound, or worsening pain.
