What should you know about gout in knuckle?

Gout is an inflammatory arthritis in which urate crystals collect in and around a joint. A knuckle may become red, warm, shiny or difficult to bend. The discomfort can be intense and may begin quickly, sometimes after a period without symptoms. Other causes of a swollen knuckle include injury, another form of arthritis, or an infection, so appearance alone cannot establish the diagnosis. A clinician may examine your hand, review medicines and health conditions, and use blood tests or joint-fluid testing when needed. Chronic kidney disease can affect urate handling, while obesity may be associated with gout risk; neither condition proves that a knuckle problem is gout.

Rest the hand, keep the knuckle raised when possible, and use a cool pack wrapped in cloth for short periods. Avoid pressing or massaging a very inflamed joint. Drink normally unless a clinician has advised fluid restriction. Do not start, stop or change gout medicines without advice, and check before taking an anti-inflammatory medicine if you have kidney, stomach, heart or blood-pressure problems, take blood thinners, or are pregnant. A clinician can distinguish treatment for an acute flare from longer-term urate-lowering care. Record when the pain began, which joints are affected, and whether you have had previous attacks.

Arrange prompt medical care for a new or unexplained hot, swollen knuckle. Seek urgent help if you also have fever, chills, feel very unwell, notice spreading redness, or cannot move the finger, because an infected joint can resemble gout and requires rapid treatment. Get immediate help after a major injury, if the finger looks deformed, or if it becomes pale, blue, numb or unusually cold.
