What should you know about gout hip pain?

Gout develops when urate crystals collect in a joint and trigger inflammation. The hip is deep beneath thick muscles, so an attack there may feel like pain in the groin, buttock, or outer hip, with reduced movement and difficulty walking. Hip pain may also come from gluteal tendinopathy, impingement, a labral tear, osteoarthritis, sciatica, or a fracture. Gout in the hip is less typical than gout in the big toe, ankle, or knee; a clinician may use examination, blood tests, imaging, or fluid testing to clarify the cause.

During a flare, reduce weight-bearing activity and use a supported position that does not aggravate the hip. Take prescribed gout treatment exactly as directed; do not start, stop, or alter medicines without medical advice. Drink fluids normally unless a clinician has limited your intake, and record when the pain began, whether the joint feels hot or swollen, and which movements are difficult. A clinician can review kidney health, current medicines, and longer-term urate-lowering treatment.

Arrange prompt medical assessment for new severe hip pain, inability to bear weight, fever, a hot swollen joint, or pain after a fall. Seek urgent help if the leg looks shortened or turned outward, or if you feel acutely unwell. A painful hip with fever can reflect an infection, which requires different treatment from gout.
